I do not accept insurance but I will provide you with an invoice if requested that you can send to your insurance company. You will need the information provided under my name above. Many insurance companies will reimburse for services rendered but it is your responsibility to check with your insurance company to see if they will cover you for my services.
I have office hours by appointment only. I am generally in the office on Wednesdays and Fridays. I will make every attempt to schedule times that are convenient for you.
Failure to keep your appointments is very costly for me. A $65 cancellation fee baring sickness or weather will be charged for missed sessions.
